As part of the USAF Global Strike Command (AFGSC) Directorate of Safety’s mission to preserve resources through mishap prevention, Synensys, LLC was contracted to perform the first-ever LOSA on High Mobility Multipurpose Wheeled Vehicles (HMMWVs).

The Synensys LOSA team performed comprehensive research on HMMWV operations through on-site visits to assess safety strengths and weaknesses. This LOSA aimed to identify threats and errors during routine HMMWV operations. Skilled observers gathered data on environmental conditions, operational complexity, and crew performance throughout these routine tasks.

Using Synensys’ proprietary LOSA safety management platform, LOSA + SafeSight®, the LOSA Team adapted and modified its existing LOSA methodology to provide a diagnostic snapshot of strengths and weaknesses that AFGSC can use to enhance safety management systems. Multiple recommendations and improvements to current processes resulted in a safer operating environment for the AFGSC airmen.
