- Operationalizing safety management on the journey to becoming a high-reliability organization requires culture change, including developing and sustaining a culture that understands that we are humans, that humans make mistakes, and that error is inevitable; a culture of open and honest communication and transparency; and a just culture with no shame or blame. This journey takes time, from 12 to 18 months for most organizations.
- Healthcare organizations need to adapt and adopt a formal threat and error management framework within their daily operations and move from reactive to proactive safety and quality.
